This Change Order #______ (the "Change Order") to Managed Services Provider Service Order #______ (the "MSPSO") is entered into by and between M-Theory Consulting Group, LLC. ("Vendor") and ______ ("Customer"), and shall amend deliverables reflected on the executed MSPSO between the parties.
1. Change of Scope. The following modifications are hereby made to the MSPSO:

NRC — Non-Recurring Charges
Monthly: $0.00 | Annual: $0.00
ITEMS TO REMOVE
2. Effect on Existing Terms. Except as expressly modified by this Change Order, all other terms and conditions of the MSPSO shall remain in full force and effect.
3. Conflict of Terms. In the event of any conflict between this Change Order and the MSPSO, this Change Order shall control.
4. Incorporation. This Change Order is incorporated into and made a part of the MSPSO.
5. Term. This Change Order shall become effective on the Effective Date specified above.
6. Entire Agreement. This Change Order, together with the MSPSO, constitutes the entire agreement between the parties.
